Day Rehabilitation Program

Spalding's Day Rehabilitation Program offers comprehensive rehabilitation services on an outpatient basis to patients who have diagnoses including, but not limited to:

  • Neurologic disorders from stroke, brain injury  or spinal cord injury
  • Amputations
  • Arthritis or rheumatic disease
  • General debilitation
  • Multiple Sclerosis, Guillian Barre and other neuropathies
  • Multiple trauma and orthopedic disorders

The Team

Day Rehabilitation Program participants have an individualized treatment plan, supervised by the physician to meet their specific needs.

The "team" may include, but is not limited to:

  • Physician(s)
  • Rehabilitation Nurse
  • Physical Therapist
  • Occupational Therapist
  • Speech-Language Pathologist
  • Respiratory Therapist
  • Therapeutic Recreation Specialist
  • Case Manager
  • Psychologist
  • The patient and family or significant other

Transdisciplinary Team Approach

This team works together to identify problem areas and specific functional interventions. The transdisciplinary approach provides a framework for patient treatment and a systematic plan for recovery. The patient and rehab staff collaborate on goal identification and timeline for completion. This collaboration leads to education, and functional outcomes.

The Treatment

The plan of treatment will be tailored to the specific needs of the participant. This could be 2-5 days per week and 4-6 hours per day. Accommodations will be provided for rest and relaxation as needed. Community reintegration activities are provided through a transdisciplinary approach to assist the participant to resume interactions with their community, work, leisure, friends and family.

The structured and supervised setting provides opportunity for patients/caregivers to identify problem areas in the home and practice solutions in a supportive and safe environment. Our goal is to assist the patient to regain optimal physical, communicative and functional abilities. Activities are provided throughout the day to complement the participants' nursing and therapy goals.

The goals of this program are to:

  • Provide a continuum of care for persons who have been diagnosed with a disabling condition and are currently experiencing complication from illness or disability
  • Decrease the inpatient length-of-stay
  • Assist participants to regain optimal physical, communicative, and functional abilities
  • Reduce complications associated with immobility
  • Lessen severity and frequency of exacerbation and to decrease emergency room visits and hospital admissions
  • Support and educate patient/family/caregiver about the disability, coping and recognizing early signs of complications
  • Decrease length of outpatient therapy program
  • Community reintegration

Meals and Medications

Nutritional meals are provided daily. Participants are welcome to bring sack lunches if preferred. Assistance is provided with medication administration as needed. Medication administration is coordinated with patient and family with a goal of self-administration, as appropriate.

Support Services

The Day Rehabilitation Program Staff is a cohesive team who recognizes that illness or disability can affect the entire family unit. Family and/or significant others are encouraged to attend therapy sessions and patient/family conferences to learn about changes and specific needs of the patient.

Education

Group and individual education and support for patients/families and caregivers can facilitate increased awareness of disability-related complications.
